Bigeminal rhythm VII.

Figure 1. Electrocardiogram recorded 14 hours after admission. See text for explication. An earlier electrocardiogram recorded immediately after admission had a sinus rate of 92 beats/min and Wolff-Parkinson-White type preexcitation on all beats. A 57-year-old woman with a history of high blood pressure and dyslipidemia presented to the emergency department with chest pain, palpitations, dyspne...

Bigeminal rhythm IV.

rhythm with each early complex having a wide QRS. Although the wide QRS superficially suggests that this is ventricular bigeminy, the perfect right-bundle-branchblock pattern (RS in lead I, qRS in lead V6, rsR ́ in lead V1) suggests that the early complex is an aberrantly conducted supraventricular complex. The atrial rhythm is typical counterclockwise atrial flutter with negative P waves in lea...

Active vs passive rhythms as an explanation of bigeminal rhythm with similar P waves.

We describe the criteria for differential diagnosis between 3:2 sinoatrial block from atrial bigeminy due to an ectopic focus in the sinus or parasinus zone. In the 3:2 sinoatrial block the RR interval of the basic rhythm is similar to the short R-R interval of the paired rhythm. In atrial bigeminy, the R-R interval of the basic rhythm is similar to the long R-R interval of the paired rhythm.

Cardiac rhythm management devices (part II): perioperative management.

IN the first installment of this two-part communication, we reviewed the indications for an implanted pacemaker or internal cardioverter–defibrillator (ICD), provided a brief overview of how a device is selected, and described the basics of pacemaker and ICD design and function. Here we discuss specific device malfunction, electromagnetic and mechanical interference, and management for patients...

Perceptual tests of rhythmic similarity: II. Syllable rhythm.

To segment continuous speech into its component words, listeners make use of language rhythm; because rhythm differs across languages, so do the segmentation procedures which listeners use. For each of stress-, syllable- and mora-based rhythmic structure, perceptual experiments have led to the discovery of corresponding segmentation procedures. In the case of mora-based rhythm, similar segmenta...
